Journeys Home

Journeys Home staff collaborate closely with the Foster Community team to support the planning and facilitation of a child's transition to their permanent home from the moment of intake. As the child approaches discharge, the Journeys Home Family-Centered Treatment (FCT) provider works alongside the team to initiate FCT services prior to discharge, ensuring a seamless continuation of care and treatment as the child transitions to their forever home.
